ASDA Special Assessment -- Online Payment Form

During the 2009 House of Delegates meeting held on Saturday, March 14, 2009 in San Diego, CA, the House voted to approve a mandatory assessment for all Active members of the Society who have been in practice for more than one year. The assessment is to help fund the organization's strategic plan. The assessment will be levied one time over three years. The first installment was due payable in June of 2009. We are currently collecting for the second installment. The following is the assessment amount based on number of years in practice:

1 - 2 years: $1,000 total assessment (payable in three annual installments of $333.33)

2 - 3 years: $2,000 total assessment (payable in three annual installments of $666.66)

3 or more years: $3,000 total assessment (payable in three annual installments of $1,000)
